FIrst TIME KinDerGarten Registration
After you have applied and received a letter from NYC DOE that you have been accepted into PS 94, you will be given an appointment to register with us. You will be sent the packet in the mail. For extra copies, to type these forms online or to download other languages. please click on the appropriate link:

You may type directly on these forms and print them. Please bring the COMPLETED AND SIGNED forms, your child's ORIGINAL proof of birth and 2 ORIGINAL proofs of address COPIES CANNOT BE ACCEPTED.
